Another case of total lack of isolation of the area during lifting:
🔸 Poorly positioned lifting personnel
🔸 Onlookers watching almost under load
🔸 Vehicular traffic allowed
In this case there were no injuries, but there was a high potential for victims!
Always require that the rigging plan specifies the isolation area.
One of the criteria for isolation radius is 1.25 X boom length + load length.
Furthermore:
🔸 No person who is not part of the lifting operation must be inside the isolation area
🔸 Only the team members necessary to carry out the lifting must remain within the isolation area
🔸 All access roads for vehicles or pedestrians must be blocked
